This meeting is an excellent team building event for not only Aerospace teams but all teams. It touches quality and safety topics by reviewing the lessons learned from the X-31 crash. It's everyone's responsibility for quality and safety. This presentation shows how the lack of communication from ground crews, engineers, to the pilot and chase pilot could have broken the link of events in which this crash could have been avoided. This NASA team was considered the A-team (the best of the best) working on this task and if it can happen to them, then it can happen to your team! From this exercise, we can all learn how to work better together in our Aerospace circles in which we can break a link that could lead to disaster!
