Vaccine FAQ:
Q: What do I do with the vaccine ticket sheet that was mailed from my municipality?
A: If you have ONE or BOTH vaccination tickets with you, we request that you cut out the sections marked in red and mail the original to the ACCJ Tokyo office by August 16 (Monday). You may choose to take a picture of it before doing so.
Q: What happens to my ticket?
A: The ticket will be pasted on your pre-screening questionnaire and sent into the vaccine vendor affiliated medical association sorted by municipality, for them to claim the vaccine inoculation subsidies.
Q: I lost my vaccine ticket.
A: Please contact your municipality to have them reissued and mail to the ACCJ. Your ticket information is necessary for your vaccination data to be inputted in the municipal system.
Q: Why was my vaccination ticket with the 1st vaccination ticket collected at the second shot?
A: We regret the inconvenience of the TKP (vaccine vendor) having returned the vaccination ticket to you without informing the ACCJ. Please mail your ticket(s) to ACCJ.
Q: I don’t have time for this. I will just pay the penalty fee.
A: We would very much appreciate your cooperation for the foreign community to assist the vaccine roll out in Japan and to be able to keep track of those vaccinated and the vaccine inventory.
Q: How do I get a vaccine passport?
A: Please refer to your municipality's website or contact them on how to apply for a vaccine passport. We understand that your record of vaccination with the date of inoculation and lot numbers are crucial information.

WHAT ARE THE FEES?
All fees for all participants you register will be charged to your ACCJ account. We are unable to accept any other form of payment at this time. The ACCJ is passing all fees to the vendor and is not charging any additional fees for this program.
Each participant will incur a charge of ¥4,400 to pay for the vendor’s administration of the program.
Actual costs for the vaccination are paid to the vendor through the Municipal Vaccination Voucher system.
If a participant does not forward their voucher to the vendor within 6 months of the second vaccination (we ask that you bring it with you to your second vaccination appointment), you will incur an additional (penalty) charge of ¥2,277 (as of June 23 and subject to increase by possibly ¥1,100). This is to compensate the vendor for their inability to recoup from the government the cost of the vaccination.
If a registered participant does not show up to be vaccinated, you will be charged the ¥4,400 fee and the penalty charge (above) for the unused appointment and vaccine. This is to compensate the vendor and the government for the unused appointment and vaccine.
As the ACCJ member, you are responsible for fees and charges incurred by each participant you register. All costs will be billed to your ACCJ account.
In order to mitigate the financial risk of pre-paying for 50-person blocks, the ACCJ will not issue any refunds after the registration deadline (6PM Sunday June 27th).

WHO IS THE VENDOR?
The ACCJ has contracted with the TKP Occupational Vaccine Center to offer members the opportunity for small group private vaccinations. More information on TKP can be found here. If you require vaccinations for more than 50 people, we encourage you to contact TKP or another third-party vendor directly.
TO RECEIVE YOUR SECOND VACCINATION:
1 prior to YOUR APPOINTMENT
Download and complete the Pre-vaccination Screener form. Note that you must use the Japanese language version of the pre-vaccination screener, but may enter your information in English.
Locate the Record of Immunization you received on-site following your first immunization. Alternatively, if you presented your municipal voucher at the time of your first immunization, bring it with you.
In either case, you must bring your municipal voucher to the second appointment if you have received it.
BRING THE PRE-VACCINATION SCREENER, RECORD OF IMMUNIZATION AND MUNICIPAL VOUCHER (IF AVAILABLE) WITH YOU TO YOUR APPOINTMENT.
2 DAY OF APPOINTMENT
Wear a mask. Wear loose-fitting clothes that allow sleeves to be fully rolled up to the top of the shoulder for vaccination. If you are feeling ill, do not attend.
Bring GoJ-issued identification to your appointment to prove your residency in Japan. This can be a driver’s license, My Number card, residence card, etc.
If you have already received a vaccination ticket/voucher from your municipality, bring it with you to your appointment. You can receive your second vaccination even if you have not yet received your ticket/voucher, but will need to make arrangements to forward your voucher to ACCJ within 6 months of vaccination.
3 arrival
Attend your appointment at the designated time (see below for address). Please be on time but do not arrive too early to limit congestion at the venue. If you have a confirmed appointment but have not yet received your timeslot, contact vaccine@accj.or.jp
Look for ACCJ staff in the ground floor venue lobby, who will confirm that you have the required forms and guide you to the venue at the correct time. ACCJ staff will be wearing large name badges with the ACCJ logo.
4 registration
You will be directed to the 10th floor at the designated time. ACCJ staff will register you, take your temperature and re-confirm your forms.
5 Medical staff review & Vaccination
A doctor will review your pre-vaccination screener form and ask any relevant questions. It is at the doctor’s discretion whether to proceed with your vaccination. Medical staff will then administer the vaccine.
6 submit documents, wait
Submit your pre-vaccination screening form and record of vaccination to TKP staff. The Record of Vaccination will be returned to you. Safeguard the Record of Vaccination document as you may need it for various reasons including receiving proposed documentation for international travel. The TKP/GoJ process requires the ACCJ to retain your pre-vaccination screener document. This document will be kept securely and matched with your first pre-vaccination screener for submission to TKP to complete the government reimbursement process.
Wait 15 minutes to ensure no adverse side-effects.
Leave the venue. Avoid rigorous exercise or excess alcohol for 12-24 hours.
ACCJ and/or TKP staff will be on-site throughout this process to guide and assist you.
Will I get my second shot at TKP (THE SAME FACILITY AS THE FIRST SHOT)?
If you have been inoculated for the first time at TKP, you will also receive your second shot at TKP.
WHEN IS THE APPOINTMENT?
Emails will be sent to all confirmed participants the week of July 19. All appointments will take place between 13:00 - 16:00 on Sunday August 1. To avoid overcrowding at the facility, you and any guests registered by you will be allocated a 15-minute interval for arrival. You will be notified of your interval by email and we are unable to change or take requests for the allocated time.

Can I add more guests to the waiting list?
The waiting list is unfortunately now closed.

The ACCJ Vaccine team is currently receiving a high number of inquiries. We will get to your email as soon as possible. We ask members to be patient, use the email address above, and try to refrain from calling our general number.
How can I access the vaccine homepage or additional information on the vaccine if I am not an ACCJ member?
The ACCJ will reach out to all registrants with more details. Please look out for an email from us.
WHICH VACCINE WILL I RECEIVE?
Moderna mRNA-1273. Please refer to this GoJ notice concerning the vaccine.
I have not received my vaccination ticket/VOUCHER yet, can I still be vaccinated?
Yes, you are able to be vaccinated without tickets . If you have received your tickets already, please bring them to the site. As long as the tickets are submitted within six months of the second shot, there is no penalty.
I only need one shot - can that be arranged?
Unfortunately, the reimbursement agreement with GoJ prevents the vendor from offering the option of only receiving one shot.
WHO IS ELIGIBLE TO RECEIVE THE VACCINE?
Anyone who is over the age of 18 and a registered resident of Japan with a registration with any municipality (juminhyo) in Japan regardless of nationality. Those that are or may be pregnant may not receive the vaccine through this particular program.
By participating in the ACCJ Small Group Covid-19 Vaccinations and signing the ‘Vaccine Release and Waiver of Liability’, do I lose the protection afforded by the Government of Japan under the national vaccination law and by extension the national Covid-19 vaccination program?
No, you will not lose the protection afforded by the GOJ under the national vaccination law. For example, the government of Japan under the vaccine law will, for those who receive the (Covid-19) vaccination, cover medical treatment fees and pension for the disabled as a result of any serious side effects.

WILL THE ACCJ BE MAKING MORE APPOINTMENTS AVAILABLE IN THE FUTURE?
We continue to support members and co-ordinate among member companies to facilitate access to vaccinations. At this time, the vendor has indicated limited to no availability until at least the end of July.
If you require vaccinations for 50 people or more, we encourage you to make arrangements directly with a third-party vaccination provider. More information can be found on the ACCJ Vaccine Information Hub.
